Commonly discussed for autoimmune conditions

Vitamin D

Low vitamin D is unusually common across autoimmune conditions and is linked to higher disease activity — in lupus specifically, deficiency has been tied to more frequent flares and higher antibody levels. Correcting a genuine deficiency through supplementation appears safe and can modestly improve fatigue and disease activity, mainly in people who were low to begin with. One long-running trial found daily supplementation reduced the risk of developing any autoimmune disease by 22% over five years. The catch is that "more is better" doesn't hold here — a blood test to confirm you're actually deficient is what makes supplementation worthwhile rather than a guess.

Omega-3 (fish oil)

Omega-3s compete with the omega-6 fats common in a typical Western diet and support the body's own processes for resolving inflammation, which is why they're studied across rheumatoid arthritis, lupus, MS, and type 1 diabetes. Dosing in research varies a lot: roughly 500–1,000mg combined EPA+DHA a day for general low-grade inflammation, up to 2,000–4,000mg a day in studies specifically on RA and IBD. Results across trials are genuinely mixed, not uniformly positive — and at higher doses, fish oil has a blood-thinning effect worth flagging if you're on anticoagulants or have a procedure coming up.

Magnesium

A common shortfall in people managing chronic illness, and one with a plausible role in gut barrier function and nervous system regulation. Early evidence pairs it with probiotics for gut and mood-related symptoms, and it shows up alongside vitamin D, B12, and omega-3s in fibromyalgia research as a reasonable adjunct — not a standalone fix.

Probiotics

Given how closely gut bacteria and immune regulation are linked, probiotics are a natural fit for conditions with a digestive component — IBD, celiac, IBS-type symptoms. Certain strains ("psychobiotics") are also being studied for their effect on mood, which matters given how often chronic illness and low mood travel together. Strain and dose vary widely between products, so this is an area where a specific recommendation from a dietitian or gastroenterologist is more useful than a generic one.

Curcumin (turmeric)

Studied across a wide range of inflammatory conditions — osteoarthritis, IBS, chronic kidney disease among them — with real anti-inflammatory signal, though study quality varies and results aren't always consistent. Most positive trials use enhanced-absorption formulations rather than plain turmeric powder, since curcumin is otherwise poorly absorbed. Worth flagging: curcumin can also have a blood-thinning effect and may interact with other medications, so it's not automatically "safe because it's a spice."

Selenium and zinc (thyroid-specific)

For Hashimoto's specifically, selenium has repeatedly been shown to lower thyroid antibody levels over 6–12 months of supplementation, and zinc is also commonly discussed alongside it. Iodine is a different story: despite being "thyroid related," excess iodine can worsen autoimmune thyroid activity and is generally something to avoid supplementing without direct medical guidance, even though it's tempting to lump it in with the others.

B12, iron, and folate

These three deficiencies show up again and again in conditions that affect nutrient absorption — celiac disease and Crohn's in particular, where B12, iron, folate (B9), and vitamin D deficiencies are all common. B12 deficiency is also frequently seen in Hashimoto's. One practical detail: iron and calcium supplements can interfere with the absorption of thyroid medication, so if you take both, they generally need to be spaced several hours apart — worth confirming the exact timing with your pharmacist.

A few rules that apply across the board

  • Get tested before you supplement where you can. Vitamin D, B12, iron, and selenium are all things a simple blood panel can confirm — guessing risks both under- and over-supplementing.
  • Check every supplement against your current medications, not just once. This matters most for blood thinners, thyroid medication, and immunosuppressants or biologics.
  • Supplement quality isn't guaranteed the way medication is — third-party tested brands (look for USP or NSF certification) reduce the risk of inaccurate dosing or contamination.
  • "Natural" doesn't mean risk-free. Iodine and high-dose ashwagandha are two examples on this page's own conditions list that can make autoimmune thyroid disease worse, not better.
